This app stores an image of a blank form, and then maps your data to the correct x-y position to fill out the form. The user can then download the PDF or print it.
Copy this Google Sheet before forking the app. Then when reconnecting the datasource, you can select your copy of the sheet. You will also have to re-link each Google Sheets query to the correct sheet after re-authorizing the datasource.
